It might be easier to find if you look under "Jelly Roll" Morton and not "Jerry".

And it is "Grandpa's Spells". I used to have a book of JRM material with this in it - I dont any more and I dont know if it is still in print. As an alternative, go here: http://www.perfessorbill.com/pbmidiall.shtml right click to download the midi file. Most midi software will turn this into a printable score.
