When someone says this do they just mean putting a bit (more) of a pause between phrases? Or are there other meanings and connotations?

I'm sure it has a lot of overlapping connotations, and I'm not nearly good enough to have a bead on all the ways in which the idea might have pianistic applications, but usually my teacher literally wants me to use my hands to exhale a phrase, almost as if I were singing the phrases and then taking a breath. Depending on the music, but certainly in bel canto type pieces, it sure can give the piece a more natural sound as you let certain expression develop over a phrase and then give some room for it to resolve while you move in to the next phrase.

Usually, yes, or maybe in other special instances, taking time in the middle of a phrase.

Sometimes, singers tell me to take a little more time at a certain point of the music, so they can take a breath without having to squeeze it in/freak out over it/inhale too rapidly.

When you get used to doing things like this, it's actually quite natural and sounds great, just like punctuation when speaking (pausing slightly where a comma would be, or at the end of a period). Of course, this can be overdone, and must be done judiciously and tastefully, but that comes with experience and learning.

I think in the end, it's all about having a natural sound, which can't be forced... And many times, barreling through cadences or arrival points in strict metronomical tempo doesn't really sound that natural.
