Please help me write an epitaph to my left hand’s 4th finger.
It’s not loyal to me (and has never been) nor it obeys my will or my desires or my intentions. Not even my dreadful practice.
I presume it’s already death and I would like to offer it a nice, polite farewell.

Wasn't that the finger, in Help! that Ringo Starr got the ring stuck on? I seem to remember a line, "I've had good times with that finger." Are you sure you want to give it up?

I'd give up skin and subcutaneous adipose tissue before giving up a finger (see my avatar).

Perhaps you've not loved it enough? And it always felt it was being neglected, compared to your other digits? Poor thing...
