(1) Do some or all pianos have a mechanism by which the speed or ease of repetition can be adjusted?

(2) If the answer to (1) is yes, why doesn't everyone just have their piano regulated for maximum speed or ease of repetition? Is there a downside in doing this?

The answer to your question is:

1)all pianos have some type of repetition mechanism. In the grands it's via the repetition lever(the more lofty sounding term "balancier" is used to designate Steinways).

In uprights, this function would be done by the backchecks but the hammer return springs play a role in this as well.

2)Any conscientious technician will adjust the above when going through the regulation steps. And no, there's really no downside. Well, perhaps, there would be in the case of grands when the hammers bounce up so aggressively that they wind up striking the strings. You could also that the cases where this upward movement might be felt by sensitive pianists who want it eliminated even if the hammers aren't hitting the strings.
